The undersigned owner(s) certifies that the information provided herein is correct. The owner(s) agrees to abide by the rules and regulations published by Cemetery and Parks Commissioners for the protection of the lot, owners, proper operation and appropriate appearance of the cemetery. A cemetery deed will be recorded at the Town Clerk’s Office. The Cemeteries and Parks Department will maintain a record of all deeds, transfers and plot cards. No transfer of any cemetery deed will be valid unless recorded at the Cemeteries and Parks Department.
